The Forum at Brookside, in Louisville, Kentucky, is a full-service retirement community that has:

- 24 personal care residences - our Personal Care Ambassador Suites
- 60 skilled/intermediate nursing beds - our skilled care Health Center

Our wellness clinic offers three modules of medical/ homemaker services to allow retirement residents to thrive longer in their familiar surroundings.
If a resident would prefer to move to our Personal Care Ambassador Suites or to our Health Center, our Resident Services Director is skilled at helping our residents and their family with the transition.
Defining Our Senior Care Levels
Independent Living
Independent Living is for the senior who wants to maintain a rich and rewarding life, without the responsibilities of running a household. The Forum at Brookside is an ideal setting for active seniors who want to focus on the things most enjoyed in life, but yet who also want to have assistance available, should it be needed.
Enhanced Services
On a temporary or permanent basis, enhanced services are available on site to residents in our independent living apartments or villas for continued independence and enhanced safety and well being. Services are coordinated by the wellness clinic nurse, and can include medication reminders, assistance with bathing or grooming, meal preparation, assistance with shopping or appointments, daily housekeeping, laundry, and personal correspondence or bill paying.
Personal Care
Personal Care means that our residents can stay in private suites, in an elegant, residential atmosphere, even though they need more assistance than our independent living residents. Personal care services can include medication administration by licensed staff, assistance with personal needs like bathing or dressing, or rehabilitation services.

Skilled Nursing
Our skilled nursing services provide 24-hour supervision and sophisticated medical services for residents who have more complex medical needs or health challenges. Our skilled nursing residents may be here for post-operative, rehabilitative or permanent care. We offer private or semi-private rooms, and rehabilitation services such as physical, respiratory, speech and occupational therapies, hydro-therapy, therapeutic dining with our on-site dietitian, and much more.
In-House Rehabilitation
Rehabilitation services right at our community is another perk of living at The Forum at Brookside. Our rehabilitation services are for both our full-time residents of our skilled nursing facility and others who need rehabilitative therapy while recovering from an illness, injury or surgery.
Short-term Stays
The Forum at Brookside also offers short-term stays for seniors who need care after a hospital stay, or when family members travel out of town or simply need a short respite break from the stresses of caregiving.
Short-term stays can vary from a week to months. Short-term guests enjoy housekeeping services, dining, community activities, transportation and other amenities, as well as coordination of any necessary medical care and help with daily living activities.
